Choose User Login
Click 'Sign Up' to create an account.
Enter your full name and your email address.
You will receive a pop-up prompting you to check your email.
You will receive an email notification to create a password.
Click on “Create a password”.
Click the eye icon to view your password.
Click 'Set Password' once it is confirmed.
You may need to “sign in” or use 'User Login' to view your ticket
Below is what you will see when you sign in.
Click 'Submit a Request' to proceed.
Enter your serial number (e.g., 5CG4142****) and click 'Prefill Details From Serial Number".
- The following information has been automatically filled in:
- Subject
- Serial Number
- Device Type
- Device Brand
- Device Model
- Registration Number
- Full Name
- Agency
- States
- Branch
Once done, click on Submit
Once submitted, you will receive a notification on the homepage: 'Your request was successfully submitted.
You also will receive a notification via email:
You will see your request appear on the page under the 'My Requests' tab
When you click on the subject, it will display all the information you entered in the web form, along with the status of the ticket.
To sign out, click on your user profile and select 'Sign Out’.
